Middle picture is Margaret's head photoshopped on actress Sherry Jackson's body from Star Trek. Who actually looks more like MJ here than Ann Margaret.

Just Googled it and found the source of the info. Interview with John Romita from Comics Creators on Spider-Man, 2001...
“When we started to plot her first appearance in Amazing #42, Stan wasn’t sure if she should be beautiful or hideous. I was recently looking through the volume of Essential Spider-Man that reprinted some of Ditko’s issues and they referred to Mary Jane as beautiful. Stan has a terrible memory and obviously forgot […] Anyway, once we agreed on making her beautiful, we had the problem of trying to make her look really spectacular. Stan wanted her to look something like a go-go girl. I used Ann-Margret from the movie Bye Bye, Birdie as a guide, using her coloring, the shape of her face, her red hair and her form-fitting short skirts. I exaggerated her dimples and the cleft in her chin.”

Related: I came across this just the other day, in Showcase #35 Gil Kane noted that "...I decided on Robert Taylor as a basis for Ray (The Atom) Palmer's face. Then I added a touch of vanilla, a pinch of salt, some pepper and a deep cleft on the chin..." Aha, I thought, that explains (Silver Age) Ray's pronounced widow's peak.

May I just say thanks for all of the pictures of Ann Margaret and Tina Louise, and that STUNNING picture of Piper Laurie. Always had a thing for redheads, so this was a nice review of some classic beauties. Now, if someone would kindly post some non-Nebula pictures of Karen Gillan, I would be forever in your debt

And that he mentions "Lion Mane" near the end there. Clearly some fanboy on the writing staff recalled Lion-Mane from the Silver Age Hawkman and slipped him in as Hawkman's arch-enemy.
